If I change a page, do I have to remember to change it in every language?

You do not have to remember: in the console the versions are linked and you see what is missing where. Each language publishes on its own, without touching the others.

Does Google penalise a translated site?

No, if it is translated well and declared well. It penalises near identical pages without hreflang, and machine translation published without anyone reading it.

Do right to left languages really work?

Yes. In Arabic the whole site flips, not just the text: menus, columns and alignment. This site is in Arabic too, you can check from the switcher at the top.
